Introduction Axillary nodal status remains a significant determinant of prognosis and indicator of need for adjuvant therapies. The move from axillary lymph node clearance (ALNC) to sentinel lymph node biopsy (SLNB) for patients with clinically negative nodes has resulted in shorter hospital stays with reduced morbidity, and is increasingly challenging our dogmas on extent of axillary surgery. There are, however, a number of contexts where the utility of SLNB remains less clearly established.
